Subscription Services Cancellation & Refund Policy
Next Level Longevity Clinic

This Subscription Services Cancellation & Refund Policy (“Policy”) explains the terms under which subscription-based services, consultations, and related healthcare offerings provided by Next Level Longevity Clinic (“Clinic,” “we,” “us,” or “our”) may be canceled or refunded. By enrolling in or purchasing any Subscription Services, you agree to this Policy.
1. Subscription Services
- Your membership in the subscription program (“Subscription Service”), including your ability to access the services, begins when you have completed the intake form at which time you will be charged for the first month of the Subscription Services.
- The Subscription Services include access to providers, medications where appropriate, and online support.
- Please note if you are disqualified by our medical provider for any medical reason you will receive a full refund.
- There is no guarantee of results or effectiveness due to your participation in the Subscription Services.
2. Cancellation of Subscription Services
- You may cancel your Subscription Service at any time for any reason. To cancel your Subscription Services, please contact customer service and request a cancellation or through the process outlined in your online account (if applicable).
- To ensure a smooth process, we require your cancellation request to be received at least 72 hours prior to your billing date. If this notice date is not met, your subscription will be charged on the next billing date, and the cancellation will take effect on the subsequent billing date.
- After cancellation, you will continue to receive the Subscription Services through the end of your billing cycle.
- Please see our Refund Policy below to determine if you are eligible for a refund of your canceled Subscription Service.
3. Refund Policy for Subscription Services
- If your Subscription Service is cancelled due to disqualification by our medical provider, you will be issued a refund for the remainder of your Subscription Service charges following the disqualification.
- Other than cancellation due to medical disqualification, IN NO EVENT SHALL YOU BE ISSUED A REFUND UPON CANCELLATION OF THE SUBSCRIPTION SERVICES.
- Federal law generally prohibits the return of prescription medications to pharmacies for refund or reuse once they have been ordered to a patient in order to protect public health and safety. This policy ensures that all medications provided are handled and dispensed with utmost care and compliance. If approved, refunds will only be issued for your most recent billing cycle and will not be issued for past billing cycles.
- Damaged or Incorrect items: Please inspect your medication immediately upon receipt. If the medication appears to be damaged or incorrect, please contact customer service immediately .
- Damaged items: Damaged items may be replaced by the pharmacy upon receipt of evidence of damage.
- Incorrect items: Incorrect medications will be replaced by the pharmacy.
4. Order Cancellation (Non-Subscription Purchases)
Non-subscription orders, including individual services or products, may only be canceled if they have not yet been processed, approved, or transmitted to a pharmacy or service provider. Once processing has begun, cancellation is no longer possible. The Clinic reserves the right to determine the status of an order at the time a cancellation request is received.
5. Medical Services and Consultations
All telehealth consultations and medical evaluations provided through licensed healthcare providers are non-refundable once completed. Missed appointments without prior notice may be non-refundable. Rescheduling may be permitted with reasonable advance notice. There is no guarantee of specific medical outcomes or results from any consultation or treatment.
6. Prescription Medications
All prescription medications are dispensed by licensed third-party pharmacies in accordance with applicable federal and state laws. Once a prescription has been approved, processed, or transmitted to a pharmacy, it cannot be canceled, returned, or refunded. This policy is required to comply with pharmacy regulations that prohibit the return or reuse of prescription medications once dispensed.
7. Shipping and Delivery Issues
We are not responsible for delays caused by shipping carriers. Delivery delays do not qualify for refunds. If a shipment is lost or damaged in transit, we will review the issue on a case-by-case basis. Supporting evidence may be required, and eligible cases may qualify for replacement at our discretion.
8. Damaged or Incorrect Items
If you receive a damaged or incorrect item, you must notify us within 48 hours of delivery. Supporting documentation, including photographs, may be required for verification. Verified cases may be eligible for replacement at our discretion.
9. Non-Refundable Situations
Refunds will not be issued for completed medical consultations, missed appointments without notice, prescriptions that have been processed or dispensed, orders that have entered processing, failure to follow provider instructions, or dissatisfaction with treatment outcomes, as individual results may vary.
10. Payment Disputes
By completing a purchase or enrolling in Subscription Services, you agree to first contact the Clinic to resolve any billing concerns before initiating a chargeback or payment dispute. 11. Subscription Services FAQs
- What is included in my Subscription Services price?
- Your Subscription Services represents a single price for the subscription purchased. This may include consultations both live and via electronic form submissions and reviews with a Provider for medications that are fulfilled through a pharmacy or other health care services on which we collect payments, lab services, care support, 24/7 patient support line and other services to support your medical weight loss journey. Services, labs and medications covered with the single Subscription Service price may vary.
- What if my provider determines I am no longer eligible for the Subscription Services?
- If your Provider determines you are no longer eligible for the Subscription Services, your subscription will be canceled, and a refund may be issued. If you have questions or concerns about an eligibility-initiated cancellation, please contact our customer service at [ï].
